Protonpompremmer bij clopidogrel: omeprazol of pantoprazol?

Samenvatting

Vraagstelling De combinatie clopidogrel en (es)omeprazol geeft volgens het NHG-Standpunt een verhoogd risico op cardiovasculaire complicaties, waardoor patiënten worden overgezet op pantoprazol. Het standpunt uit 2010 is gebaseerd op farmacologische onderzoeken, omdat klinische onderzoeken wisselende resultaten lieten zien. (Es)omeprazol remt het CYP2C19-enzym, dat clopidogrel in zijn actieve metaboliet omzet, waardoor de plasmaspiegel van clopidogrel halveert en de remmende werking op plaatjesaggregatie met 30% vermindert.1 Bij pantoprazol zou deze interactie niet optreden. Is er een verschil in cardiovasculair risico tussen omeprazol en pantoprazol in combinatie met clopidogrel?
