This thesis starts with a study on voluntarily reported records of medication errors in 11 Dutch ICUs, giving insight into the scope of the medication safety problem in the ICU.
Next the development and effect of two medication safety practices [a clinical pharmacy service and a medication reconciliation service] by a dedicated clinical pharmacist in the ICU, is presented.
Finally, we describe the development of a prediction model for the identification of patients at risk of medication transfer errors at ICU discharge.
