The main objectives of this thesis are to identify determinants for bleeding in hospitalized patients treated with anticoagulant therapy and to characterize anticoagulation-related medication errors (part 1).
In addition, to determine the effect of antithrombotic stewardship on bleeding complications and thrombotic events and adherence to anticoagulant guidelines among prescribing physicians (part 2).
