In this chapter the aetiology and pathogenesis of psoriasis are discussed. Accumulating evidence indicates that psoriasis is a multifactorial disorder caused by the concerted action of multiple disease genes in a single individual, triggered by environmental factors. In the past, abnormalities in keratinization were considered the etiopathogenic basis of psoriasis; however, it is now currently thought that it is initially an immune affection mediated by type Th1 response. Recently, new data have led to the assumption that antigen-presenting cells are important not only in the induction of psoriasis but also in its maintenance.
