We present the case of a 60-year-old woman with one a year history of erythematous papules bilateral in the submammary region. The combination of the clinical presentation and the histopathological findings led us to the rare diagnosis of granular parakeratosis. Granular parakeratosis is a benign disorder of keratinisation. Most patients are women in or above their 40's with skin lesions in intertriginous areas. There also exists an infantile form in children wearing diapers. Possibly is granular parakeratosis a reaction pattern of the skin that can be triggered by different factors, such as excessive use of deodorants, dermatophytosis or mechanical factors. Granular parakeratosis is a chronic disease that may resolve spontaneously after months to years. Beneficial effect of retinoids is described in the literature.
