Popliteal venous aneurysm is a rare diagnosis, associated with a high risk of pulmonary embolism in 50 % of cases, sometimes with fatal consequences. Surgery can dramatically reduce the risk of pulmonary embolism. Dermatologists may coincidentally discover such an aneurysm when examining the popliteal fossa. We report a 66-year old man with recurrent pulmonary embolisms in the past and a giant popliteal venous aneurysm (9.3 × 4 × 4 cm).
