We report a case of an 83-year old woman with chronic venous insufficiency and repeated bleeding caused by pulsatile varicose veins related to severe tricuspid insufficiency. She was suffering from pain in the legs with extensive varicose veins and had several episodes of varicose bleeding. Conservative treatment and attempts to perform ambulatory phlebectomy failed. Duplex ultrasound revealed the presence of pulsatile varicose veins and the relationship with underlying tricuspid insufficiency became obvious.The patient was eventually treated successfully by a cardiologist and a dermatologist.
