Hidradenitis suppurativa (HS) is a chronic, debilitating inflammatory disease of the skin and is characterized by painful inflamed nodes, abscesses, fistulas, and skin fibrosis with hypertrophic scars. It typically manifests between the ages of 10 and 30. It is primarily a disorder of the hair follicles, involving immune dysregulation. It is not an infectious disease and its pathogenesis is different from that of acne. Risk factors are smoking, overweight, and a family history. Compared with many other chronic skin diseases, HS has a significant impact on quality of life. Treatment consists of local, systemic, or surgical interventions, depending on the severity and extent of the disease. Remission is often achieved with a combination of anti-inflammatory biologicals and surgery. If general practitioners recognize HS in an early stage, treatment can be started early, thereby limiting the spread of the disease and the development of complications. While mild HS-related symptoms can be treated in primary care, GPs should refer patients to a dermatologist if treatment is insufficiently effective or if the disease is severe. GPs also have a role in managing risk factors and the psychosocial consequences of the disease.
